Bowtie ; Steriliser - practical training materials
Sterilizer bowtie into practical training materials you can use immediately in your mill under Process Safety Management.
🧾 1. TRAINING POSTER
You can place this in control room / sterilizer area 👇
🚨 STERILIZER SAFETY – HIGH PRESSURE ZONE
⚠️ TOP RISK
Sterilizer Overpressure / Door Failure / Explosion
🔴 BEFORE OPERATION (PREVENTION)
✔ Check pressure gauge working & calibrated
✔ Ensure drain line NOT blocked
✔ Confirm door fully locked
✔ Verify interlock system ACTIVE (never bypass)
✔ Check steam valve condition
✔ Follow correct steaming SOP
🟢 DURING OPERATION
✔ Monitor pressure continuously
✔ Do NOT exceed safe pressure limit
✔ Listen for abnormal sound / vibration
✔ Ensure no leakage at door or pipeline
✔ Stay within safe distance
🔵 AFTER OPERATION
✔ Fully depressurize before opening door
✔ Open door slowly (confirm zero pressure)
✔ Inspect for damage / leakage
✔ Record operation in logbook
🔥 EMERGENCY ACTION
❗ Close steam supply immediately
❗ Activate emergency isolation
❗ Evacuate area
❗ Inform supervisor
❗ Follow ERP (Emergency Response Plan)
⚡ GOLDEN RULES
❌ NEVER bypass interlock
❌ NEVER open under pressure
❌ NEVER ignore alarm
✅ 2. OPERATOR CHECKLIST (Daily Use)
🟡 A. Pre-Start Checklist
Pressure gauge OK
Safety valve (PRV) tested/valid
Drain line clear
Door locking system OK
Interlock system functional
No steam leakage
SOP available & understood
🟠 B. Operation Checklist
Pressure within limit
No abnormal noise
No vibration/leakage
Cycle follows SOP
Operator present at panel
🔵 C. Shutdown Checklist
Pressure = 0 before opening
Steam isolated
Door opened safely
Equipment condition checked
Logbook updated
🔴 D. Emergency Checklist
Steam valve closed
Area evacuated
Alarm raised
Supervisor informed

💡 Practical Tip (Very Important)
In many mills, accidents happen not because of equipment failure—but because:
“Boleh lagi…” mindset
Rushing production
Ignoring alarms
👉 Your checklist should be signed per shift to enforce discipline.

Bowtie ; sterilizer at palm oil mill
🌴 Bowtie: Sterilizer Overpressure / Explosion
🔷 Visual Concept
🔶 1. 🎯 Top Event (Center)
💥 Sterilizer Overpressure / Door Failure / Explosion
🔶 2. ⚠️ Threats (Left Side – Causes)
Typical real issues in mills:
Steam inlet valve stuck open
Pressure control valve failure
Blocked condensate / drain line
Faulty pressure gauge (wrong reading)
Door not properly locked
Interlock system bypassed
Operator error (wrong steaming sequence)
🔶 3. 🛡️ Preventive Barriers (Left Controls)
To prevent the top event:
Pressure relief valve (PRV) – tested regularly
Interlock system (door cannot open under pressure)
Routine calibration of pressure gauges
Steam control valve maintenance
SOP for sterilization cycle (charging, steaming, venting)
Operator training & competency certification
Drain/condensate line inspection
🔶 4. 💣 Consequences (Right Side)
If failure occurs:
Fatal injury (door burst / steam release)
Severe burns to workers
Equipment destruction
Production downtime
Legal & regulatory action
🔶 5. 🚨 Mitigation Barriers (Right Controls)
To reduce impact:
Emergency steam isolation valve
Pressure venting system
Safety distance / barricade around sterilizer
Emergency response plan
PPE (heat-resistant gear)
First aid & medical readiness
Incident investigation system
🔷 Simple Bowtie Layout (Sterilizer)
Threats Prevention TOP EVENT Mitigation Consequences
-------- ---------- --------- ---------- ------------
Valve failure →Valve maintenance→Sterilizer Explosion →Emergency isolation→Fatality
Blocked drain →Line inspection → → Pressure venting → Severe burns
Gauge faulty →Calibration → → PPE → Injury
Door unlocked→Interlock system → → Barricade → Equipment damage
Human error → SOP & training → → ERP → Shutdown
🔥 Critical Real-Life Lesson (Palm Oil Mill)
Most sterilizer accidents happen due to:
Interlock bypassed
Operator rushing cycle
Poor maintenance of PRV
👉 The danger is not visible — pressure builds silently.
✅ Practical Tip for Your Mill
Turn this bowtie into:
Control checklist for operators
Audit checklist for management
Training poster in control room
Bowtie ; Palm Oil Mill Boiler example
Here’s a practical Bowtie example specifically for a palm oil mill, commonly applied in Process Safety Management.
🌴 Bowtie Example: Boiler Explosion in Palm Oil Mill
🔷 Visual Example
🔶 1. Top Event (Center)
🔥 Boiler Overpressure / Explosion
This is one of the highest risks in a palm oil mill due to steam generation for processing.
🔶 2. Threats (Left Side – Causes)
Typical causes in a mill environment:
Low water level in boiler drum
Failure of feedwater pump
Blocked steam outlet
Safety valve malfunction
Operator error / poor monitoring
Scale buildup (poor water treatment)
🔶 3. Preventive Barriers (Left Controls)
Controls to prevent the top event:
Automatic low-water cut-off system
Routine boiler inspection & maintenance
Water treatment program
Pressure relief valve testing
Operator competency training
Alarm & interlock system
🔶 4. Consequences (Right Side)
If explosion occurs:
Fatalities / severe injury
Major plant damage
Production shutdown
Fire outbreak
Environmental impact
🔶 5. Mitigation Barriers (Right Controls)
Controls to reduce impact:
Emergency shutdown system (ESD)
Explosion relief panels
Fire protection system (hydrant/sprinkler)
Emergency response plan
PPE & evacuation drills
Insurance / business continuity plan
🔷 Simple Bowtie Layout (Palm Oil Mill Context)
Threats Prevention TOP EVENT Mitigation Consequences
-------- ---------- --------- ---------- ------------
Low water level → Level control → Boiler Explosion → ESD system → Fatality
Pump failure → Pump maintenance → → Fire system → Plant damage
Scale formation → Water treatment → → Evacuation plan → Shutdown
Human error → Training/SOP → → PPE → Injury
🔷 Another Common Palm Oil Mill Bowtie (Quick Idea)
🛢️ Digester / Press Fire
Top Event: Oil ignition
Threats: Overheating, friction, poor housekeeping
Prevention: Temperature control, cleaning, lubrication
Mitigation: Fire suppression, extinguishers
🔥 Practical Insight (From Industry)
In many palm oil mills, the biggest gap is not equipment — but discipline:
SOP not followed
Alarm ignored
Maintenance delayed
That’s where bowtie helps — it makes everyone SEE their role in safety barriers.

Survival
From Survival to Strength: A Relentless Journey
Since childhood, life was never easy for me. I was not the kind of student people admired—my academic results were far from outstanding. Even in matriculation, I had to repeat. University was no different; I struggled, failed twice, and came dangerously close to being expelled.
But I held on.
By the grace of God, I finally graduated—though it required an extra semester, and my final CGPA stood at 2.18. To many, that number might seem small. To me, it was a symbol of resilience.
I knew one thing clearly:
If I wasn’t naturally gifted, I had to outwork everyone else.
So I did.
The Decision to Work Harder Than Everyone Else
Right after graduation, I didn’t wait. I stepped straight into the workforce—starting in a steel factory, then moving through several industries. I was fully aware of my limitations. My results were average, but my determination had to be extraordinary.
I pushed harder.
Harder in finding opportunities
Harder in learning
Harder in proving myself at work
Eventually, I entered the palm oil industry—a path that would demand everything from me.
And I gave it everything.
The Sacrifices No One Sees
While others sought salary increments, I chose a different path.
I invested in myself.
Every extra ringgit I had went into learning:
Steam Engineer certification
Internal Combustion Engine certification
DIPOM
MBA
Communication training through Toastmasters
And countless other competencies
I sacrificed time.
I sacrificed comfort.
I sacrificed many things people take for granted.
I told myself:
“It’s not my time to demand yet. It’s my time to build.”
The Breakthrough
In less than three years, something unexpected happened.
I was entrusted to become a Plant Manager.
Not because I was the smartest.
But because I was prepared.
From 2006 until today—2026—I have been managing large organizations. Leading people. Making decisions. Carrying responsibilities many never see.
A 20-Year Promise Fulfilled
One goal remained unfinished for a long time: becoming a Professional Engineer.
It stayed in my bucket list for years—delayed, postponed, set aside.
But this year, after 20 years, I finally achieved it.
Am I Successful Now?
Am I wealthy?
Am I living a life of luxury?
No.
I am still in survival mode.
Still fighting. Still pushing forward.
But when I look back…
I smile.
Because I made it through everything that once felt impossible.
And the Journey Continues
This is not a story about instant success.
This is a story about endurance.
About showing up when it’s hard.
About choosing growth over comfort.
About believing in yourself—when there is no evidence yet.
I am still climbing.
And I will keep climbing.
Perjuangan
“Dari Hampir Gagal ke Terus Berjuang”
Catatan seorang insan yang tidak pernah berhenti belajar
Sejak kecil, hidup saya bukanlah kisah seorang pelajar cemerlang.
Harga Sebuah Perubahan
Apabila saya memasuki industri sawit, hidup saya berubah sepenuhnya.
Saya mula berkorban.
Korban masa
Korban keselesaan
Korban kehidupan sosial
Korban banyak perkara yang orang lain anggap normal
Setiap hujung bulan, duit lebihan bukan untuk keseronokan.
Ia dilaburkan kembali kepada diri sendiri.
Saya belajar tanpa henti:
Sijil Jurutera Stim
Jurutera Enjin Pembakaran Dalam
DIPOM
MBA
Komunikasi melalui Toastmasters
Dan pelbagai lagi kompetensi
Ketika rakan-rakan sibuk meminta kenaikan gaji, saya memilih jalan yang berbeza.
Saya berkata pada diri sendiri:
“Belum masanya untuk saya demand. Masanya sekarang adalah untuk membina.”
Rezeki Tidak Pernah Salah Alamat
Belum pun tiga tahun bekerja…
Saya diberi amanah menjadi pengurus kilang.
Bukan kerana saya paling pandai.
Tetapi mungkin kerana saya paling bersedia untuk belajar.
Dari tahun 2006 hingga 2026 — 20 tahun perjalanan — saya terus memimpin, mengurus, belajar, jatuh dan bangkit semula.
Impian Yang Tertangguh
Dalam banyak senarai impian hidup saya, ada satu yang sering tertangguh:
Professional Engineer (PE)
Bukan sekali saya cuba, tetapi berkali-kali saya tangguhkan.
Kesibukan kerja. Tanggungjawab. Kehidupan.
Namun, awal tahun ini… selepas 20 tahun, akhirnya saya berjaya mencapainya.
Bukan kerana ia mudah.
Tetapi kerana saya tidak pernah benar-benar berhenti.
Adakah Ini Penghujung?
Adakah saya kini sudah senang?
Sudah kaya?
Sudah terbilang?
Jawapannya: Tidak.
Saya masih dalam mode survival.
Masih berjuang.
Masih belajar.
Tetapi apabila saya menoleh ke belakang…
Saya tersenyum.
Kerana saya tahu,
saya bukan lagi orang yang sama seperti dahulu.
Penutup: Sebuah Perjalanan, Bukan Destinasi
